Preventative services – Your first line of defense and foundation for wellness 

There are no restrictions on where or who you see. Simply take a photo of your expense, submit it, and be reimbursed within 48 hours. Here’s what’s included: 

  • Annual Wellness Exams: Up to $4,500 per person, per year. 
  • Well Child Exams – Comprehensive coverage for preventive care. 
  • Gyn Well woman exam – including pap 
  • Mammogram including breast ultrasound or MRIfollowing a positive mammogram after age 40 
  • Breast Thermography after age 40 
  • Derm Skin checks 
  • Cancer Screenings: Including mammograms, ultrasounds, MRI, thermography, colonoscopy, and Cologuard. 
  • Bone Density Tests and Blood Work: Lipid panel (cholesterol and triglycerides), Comprehensive Metabolic panel (CMP), Complete blood count with differential test, Thyroid Panel, Routine comprehensive Urinalysis, Hemoglobin A1C, Vitamin D level 
  • PSA screening for men 50 or earlier with family history 
  • Bone Density every 2 -3 years for women post menopause 
  • Vaccines: Flu, Shingles and Pneumonia vaccine 
  • Vision Exams: Up to $250 per person, per year. 
  • Dental Exams and Cleanings: Up to $250 per person, per year.

Larger medical needs: 

  • No restrictive networks 
  • You choose Intial Unshared Amount: 1k, 2.5k or 5K 
  • Members may choose  $1000, $2500 or $5000 IUA (like an insurance deductible) 
  • Initial Unshared Amount represents the total of any new accident, illness or injury 
  • Members are “self pay patients” up to the point of the Initial Unshared Amount (IUA) 
  • If a member anticipates the IUA being reached, they should begin a “Needs Case” 
  • Medical needs are shared in full after the Initial Unsharable Amount (IUA) is met 
  • The Needs Case remains open until no expenses have been submitted for 12 months  
  • A membership would never have more than 2 open Needs cases.  
  • Any Need above 3 needs in any 12 month period would be shared in full after a $500 threshold 
  • No Annual, lifetime, or maximum upper sharing limit 
  • Supportive of Alternative Health when a treatment plan is submitted & approved. Naturopaths, etc. included.
